Our nurses develop a comprehensive Personal Prevention CareTM plan for these older adults that guides, motivates and simplifies taking medications to enable them to live independently and avoid hospital stays.
*Patients who do not qualify for GUIDE dementia services may have a small copay for Personal Prevention Care

Pharmacy Services
Your personal pharmacist specializes in working with older adults like you who have many prescriptions. You can call them to ask questions or discuss any issues you may be having with your medications. Your pharmacist will also look for opportunities for generic substitutions and removing any drugs with the doctor’s permission.
Your nurse practitioner will discuss your medications with you and identify any issues that need to be addressed with your pharmacist or physician.

Monitoring and Tracking
When your health indicators are tracked and recorded, it enables your care team to more accurately assess your progress and status, enabling better care. This monitoring also enables your nurse to make quicker adjustments to your care plan when required.
Easy to use systems can also be provided for daily monitoring of specific indicators such as blood pressure, weight and blood oxygen levels.
