Dementia is a difficult disease to manage, whether you are living with dementia or a caregiver. Medicare has started the GUIDE Model with no copays or deductibles to help both caregivers and their loved ones. You do NOT have to change doctors to participate in GUIDE.
- Embrace provides a comprehensive assessment and develops a personalized care plan.
- We provide skilled care navigators to provide care for both those living with dementia and their caregivers.
- Your care navigator coordinates your care with your primary care doctor and any specialists.

In recognition of the value caregivers provide in care, Medicare will cover $2,625 in respite care annually to support qualifying caregivers.

Our Medicare-covered nurse practitioners use Personal Prevention CareTM to help older adults focus on the most important steps to manage dementia and other chronic conditions so they stay healthy, independent and living in their homes.
Personal Prevention CareTM improves health through:
- Frequent interaction with a personal nurse practitioner
- Consultations with pharmacists who specialize in treating older adults with complex health problems.
- Tracked progress across multiple dimensions to identify potential problems before they get serious.

Medicare Pays for Monthly Care
Older adults with dementia, heart failure and other chronic conditions may be eligible for additional Medicare paid services. Our nurses develop a comprehensive care plan to help guide, motivate and simplify taking the right actions and medications to live independently and avoid hospital stays.
