Local nurse practitioners work with you and your doctor to develop a personalized path that allows you to remain independent and hospital-free. Our pharmacy works with each of your doctors to build a comprehensive list of your prescriptions, evaluate them and our nurse practitioners work with you to eliminate confusion and missed meds. We track your indicators which allows us to determine that your health remains on track and provides early warnings when complications arise.

Embrace pharmacists consolidate all of your meds from all of your care providers and evaluate them to ensure that all are required and at the proper dosages. Our pharmacists are available to answer your questions, and our nurses ensure that you take your medications as intended.

Pharmacy Services
Taking all your medicines as prescribed is a key component to staying hospital-free.
Your personal pharmacist is only a call or email away and ready to address your questions and concerns 24 x 7. We specialize in medicines for older adults just like you – with multiple chronic conditions requiring multiple prescriptions.
Your nurse also helps out, helping you identify and overcome any barriers that may keep you from taking your meds on schedule. In addition, they communicate your complete drug listing to each of your care professionals.

Tracking Health Status
Tracking and interpreting key health indicators allows for better decisions and faster progress. Your nurse will identify personal indicators that are specific to your health status and your chronic conditions.
Over time, these indicators show trends that highlight the direction of your health and the results of you taking active control.
Your progress reports will be shared with all of your care providers to provide them with the most current information, enabling them to provide more informed care.

The majority of the costs for Personal Prevention CareTM are covered by Medicare; you are responsible for a small copay, usually less than $10.00 per month. In many instances those copays are covered by your Medicare Advantage or supplemental insurance plan.

True Cost of Hospitalization
Even though much of the cost of a hospital stay is picked up by Medicare, your out-of-pocket costs could be thousands of dollars. If you are admitted to the hospital on an observational basis, and then discharged to a care facility, your portion can rise to almost $5,000.
When you use Embrace’s Medicare-covered services, you are required to make a monthly copay. In most instances, this payment is less than $10 per month or about $ 100 per year, less than a tenth of the cost of the least expensive hospital admission, and often is covered by Medicare Advantage or supplemental insurance plans.
The unfortunate truth is after any hospital stay, older adults’ overall health is often worse after they leave than before they entered. Additional time and effort is required just to get the patient’s health back to where it was prior to admission.
