Under Medicare's GUIDE Model
Dementia is a difficult disease to manage, whether you are a patient or a caregiver. This is why Medicare has started the GUIDE program with no copays or deductibles to help dementia patients and their caregivers. You do NOT have to change doctors to participate in GUIDE.
- Embrace provides a comprehensive assessment and develops a personalized care plan.
- We provide skilled care navigators to provide care for both patients and caregivers.
- Your care navigator coordinates your care with your primary doctor and any specialists.
- In recognition of the value caregivers provide in patient care, Medicare will cover $2500 of respite care annually to support qualifying caregivers.

Ongoing Monitoring and Support
Care Navigators provide guidance via frequent telehealth visits in the comfort of your home.

Comprehensive Assessment for Each Patient
Forms the basis for a personalized care plan.

Respite Services Provide Support for Caregivers
Qualifying caregivers are supported with temporary respite services of $2,500 per year paid by Medicare.

Coordination of Entire Care Team
Care team coordination improves patient care and reduces caregiver burden.

Assistance Finding Support Services
Our team connects you to vetted community support services that you need.

Medication Management and Reconciliation
Ensures the right medications are taken at the right times.
All GUIDE services are provided with no cost or copayments for qualifying Medicare beneficiaries.
GUIDE allows us to have a dedicated Care Navigator work regularly with you and your caregiver virtually to address your specific needs, answer your questions and coordinate your care to help keep you safely in your home.
Eligibility for participation in GUIDE is open to beneficiaries with diagnosed dementia and who have Traditional Medicare as their primary insurance. At this time, GUIDE is not available for patients with Medicare Advantage, those residing in a skilled nursing facility, or patients enrolled in PACE or hospice programs.
