Tracking Progress
Local professional nurses work with your loved one and their doctor to develop a personalized prevention care plan that helps them remain hospital-free, independent and safe. Our pharmacy works with each of your older adult’s doctors to build a comprehensive list of prescriptions and our nurses ensure that medications are taken correctly. We track your loved one’s vitals which allows us to determine that their health remains on track and provides early warnings when complications arise.
Personal Prevention CareTM
Your nurse works regularly with your loved one to motivate and encourage the right steps that help them follow their individualized health plan and achieve their goals.
Older adults with dementia and multiple chronic conditions typically take six to twelve medicines every day, and sometimes more. Our pharmacists make sure they are taking the right drugs, and both our pharmacists and nurses work to make it easy to take the right meds at the right time every time.
Monitoring the right health indicators allows your nurse to identify problems early, and work with your loved one’s physician to take actions to help avoid a health issue that requires a hospital stay.
Personal Prevention CareTM provides you with the peace of mind knowing that your loved one is being well taken care of, allowing them to live safely and age in place.
Local, Professional Nurses
As a trained medical professional specializing in older adults with dementia and chronic conditions, your nurse understands the specific issues and concerns of your loved one. They use their knowledge and exclusive Embrace systems to provide guidance and care.
Your Embrace nurse motivates and encourages your loved one to follow their care plan and achieve their health goals, in ways that may often be difficult for you to do.
Our nurses coordinate care with the other members of the care team, including you. By ensuring that each contact with your older adult is appropriately documented and that key health indicators are recorded, these records can be shared with the other health professionals caring for your loved one. This coordination of care improves outcomes because everyone is working from the best, most up-to-date information.
Trained nurses along with your loving care will keep your older adult healthier and hospital-free.
Pharmacy Services
Embrace consolidates prescriptions from all of your loved one’s doctors and creates a complete medication list which is then shared among all caregivers. This ensures that all are up to date and working together.
Our pharmacists are available for consultation, and they work to substitute less costly generic drugs when available and see if there are opportunities to remove drugs. Because our pharmacists work exclusively with older adults, we have a deep understanding of what typically works best for your loved one’s specific set of conditions.
Your nurse practitioner reviews medications with your loved one to ensure that the drugs are being taken properly. If side effects are a problem, your nurse can work with the pharmacist to identify possible alternatives and recommend these to the prescribing physician.
Embrace’s pharmacy services make certain that each person gets the full benefits from their medications.
Monitoring and Tracking
By monitoring key health indicators, your nurse tracks the health progress of your older adult, allowing quicker adjustments to their care plan when needed.
When there are changes, the monitored indicators are shared with the appropriate physicians to improve their ability to analyze the current situation, and prescribe any changes.
If required, Embrace has additional monitoring solutions which can enhance physical safety too. Each of these solutions is designed to meet your loved one at their ability and skill level. Ask about our options today.
Embrace monitoring and tracking provide for timely adjustments to the health plan to avoid hospitalizations.
Covered by Medicare
For patients multiple chronic conditions who don’t qualify for GUIDE, Medicare covers the majority of the costs for your Embrace nurse. There may be a small copay, usually less than $10.00 per month. In many instances the copays are covered by Medicare Advantage or a supplemental insurance plan. If you have questions about costs, don’t hesitate to call, we’ll make sure there are no surprises.
Medicare is willing to pay for these services because the regular interactions with a nurse and monitoring have been proven to improve health and reduce hospital and emergency room visits.
Improve the safety of your loved one and their quality of life with Personal Prevention CareTM covered by Medicare.
Remain at Home
When older adults regularly follow a customized care plan the odds that they will experience a health emergency are minimized. Actively working on healthy initiatives with a nurse maintains health.
As the nurse tracks your loved one’s important health indicators, it becomes easier to identify problems early. That early identification allows doctors to get involved and often avoid a trip to the hospital.
In those situations where hospitalization is required, the active measures and regular monitoring means that conditions are likely to be less severe. Our nurse provides the latest information to the hospital and doctors enabling better decisions that shorten the hospital stay. Post discharge, frequent nurse interaction can also reduce the chances of readmission.
When older adults stay out of the hospital, their quality of life is improved and they can continue to safely age in place.
True Cost of Healthcare
Even though much of the cost of a hospital stay is picked up by Medicare, out-of-pocket costs could be thousands of dollars. If your loved one is admitted to the hospital on an observational basis, and then discharged to a care facility, those costs can rise to almost $5,000.
When you use Embrace’s Medicare covered services, you are required to make a monthly copay, although supplemental insurance may cover it. In most instances, this payment is less than $10 per month or about $100 per year, less than a tenth of the cost of the least expensive hospital admission.
After most hospital stays, older adults’ overall health never quite recovers to the point it was before the event that caused the hospital stay. Often what happens is a recurring cycle of declining health and hospital stays until the hospital discharge is to a long-term care facility which the individual never leaves.
For this reason, even with the added copays, healthcare designed to provide better care, avoid hospital stays and age in place as long as possible is the least costly path.
Coordinated Care
When the latest health conditions are regularly communicated with all members of the care team, each member is more fully informed and makes better decisions, which leads to more positive health outcomes.
Finally, when the Embrace nurse has the responsibility of keeping all members of the care team informed, including you, families get a much better understanding of the full health picture for their loved one.
Coordinated care and communication between all the parties improves care providing the best quality of life possible.
